29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a. On or about 03/23/2017 at 205 15th Ave SE Saint Petersburg, Fl 33701, an employee suffered a work-related injury and was hospitalized for surgery. The employer was aware of the in-patient hospitalization on 03/23/2017 at 10:30 pm. The employer notified OSHA on 03/27/2017 at 4:03 pm of the in-patient hospitalization.
