At this establishment, employees exposed to energy sources such as compressed air, electricity and hydraulics had not been trained. a). 29 CFR 1910.212(a)(1): The employer did not ensure that one or more methods of machine guarding were provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a). In the Mix Room: On or about May 9, 2024, and at times prior, the employer permitted employees to circumvent machine guarding at the Mixer machine by removing the barrier guard on the running machine to add gear oil and remove debris from the machine.

29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly, which applies to any extinguishers provided in the workplace. a) Warehouse: On or about May 9, 2024, and at times prior, the employer exposed employees to fire hazards, in that, monthly fire extinguisher inspections were not conducted to ensure that all fire extinguishers were working properly throughout the facility. Fire extinguishers were located in various areas throughout the warehouse.
