Mixer Building - On or about and times prior to March 28, 2017, the top guardrail system members (i.e. metal linked chains) equipped around two sides of the Additive Deck elevated platform ranged from 29 to 36 inches in height, thus exposing employees to a fall hazard.

29 CFR 1910.29(b)(2): The employer did not ensure that midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high. a). Mixer Building - On or about and times prior to March 28, 2017, two sides of the Additive Deck elevated platform were not equipped midrails or other equivalent intermediate members, thus exposing employees to a fall hazard. b). Mixer Building - On or about and times prior to March 28, 2017, there were two ladder entryways on the mixing deck that were not equipped with midrails or other equivalent intermediate members, thus exposing employees to a fall hazard.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section. a). A and B Shops - On or about and times prior to March 28, 2017, the written energy control procedures for the UPM, TRIA, and SICA Bellers did not identify all energy sources (i.e. pneumatic and hydraulic) and the techniques necessary to control the hazardous energy sources, thus exposing employees to a programmatic hazard.
