This condition exposed employees to struck-by and caught-in hazards.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) On or before May 4, 2025, and at times prior, the employer did not ensure the SICA saw and puller on line 1 were shut down properly using the procedures established for the machine once the barriers were removed and the pipe became jammed. The procedures in place were not utilized nor adequate, exposing employees to struck-by and caught-between hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or before May 4, 2025, and at times prior, the employer did not ensure that guards on the SICA saw were securely attached to prevent employee exposure to hazardous moving parts while in operation. Guards were installed with access tabs rather than bolts, in violation of the manufacturer specifications. This condition exposed employees to caught-between hazards.
