29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a) Production Area, Finished Goods Scale 301: On or about March 25, 2017, and at times prior, the employer exposed one employee to caught-in hazards while cleaning an energized rotary valve prior to receiving authorized Lock-out/Tag-out training.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition.  a) Plant, staging area: On or about April 3, 2017, and at times prior, the employer exposed employees to struck-by hazards in that a forklift was used with a damaged hydraulic hose when moving material.
