29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    (a) Location - 479 Canal Street, Somerville, MA:  On or about 3/29/2017, an employees was utilizing an electrical extension power cord which did not have a path to ground that was permanent and continuous. (Ground pin missing)     MacCormack Plumbing, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.404(f)(6), which was contained in OSHA inspection number 954831, citation number 01, item number 001, and was affirmed as a final order on 5/9/2014, with respect to a workplace located at 1/14/2014.
