29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Americas Finest Trawler, Pilot House; A Robson Heater was plugged into a surge protector (relocatable power tap). The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not heaters or other appliances. Also, relocatable power taps are designed for office types of environments, and not construction/shipbuilding environments which can get dirty/dusty during work operations. This exposes employees to possible electrical fire hazards.

29 CFR 1915.73(b): When employees were working in the vicinity of flush manholes and other small openings of comparable size in the deck and other working surfaces, such openings were not suitably covered or guarded to a height of not less than 30 inches: a) Americas Finest Trawler, Freezer Room; Employees were working near holes which contained recessed covered manholes. These holes measured approximately 32 inches by 31 inches wide and 8-12 inches deep.

29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2): a) Americas Finest Trawler, Freezer Room; Employees were exposed to an unguarded elevator pit measuring approximately 8 feet 9 inches wide and 5 feet 5 inches at the deepest point.
