29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided:    On or about April 4, 2017 employees working in the welding room were exposed to electric shock hazards and the employer did not ensure that equipment was free of damage:    a.  The faceplate on a 110 volt electrical outlet on the south wall was damaged and afforded access to live conductors.    b. Two 220 volt outlets on the north wall used to power welders were missing the plastic cover and live conductors were exposed.    c. One welder plug on the north wasll was damaged and plugged in to a live 220 volt outlet.
