29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about April 4, 2017, the Employer's written Lockout/Tagout program did not include machine specific energy control procedures for the benefit of employees servicing and/or performing maintenance on equipment such as but not limited to the Mira-Pak sealing machines that had pneumatic and electrical energy. Failure to do so exposed employees to the hazard of contact with the point of operation if the machine was unexpectedly energized.

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used in continuous lengths without splice or tap: On or about April 4, 2017, and times prior thereto, in the Warehouse Packing Department, Mira-Pak Bagger A sealing machine, the cord coming from the back of the motor for the feeder was spliced to a yellow #12 heavy duty cord that was wired to a 20 amp breaker. Employees were exposed to an electrical shock hazard.
