29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about July 16, 2018 and times prior thereto, in the warehouse department, the guarding on the Mira-Pax "C" machine was not adequate to prevent the operator's hands from coming in contact with the sealing jaws of the machine. This condition exposed employees to amputation and burn hazards.
