29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    (a) Watson Mills and Design, LLC. at 1589 S. 120th St, Lafayette, CO: On and before April 5, 2017, the employer did not ensure that the manually adjusted guard on an Altendorf F45 Sliding Table Saw provided protection equivalent to the protection of automatically adjusted guards in that the blade guard was positioned by the operator slightly above the height of the cut but was not lowered to fully enclose the blade following the cut and prior to additional cuts at the same wood height. This condition exposed employees to laceration and amputation hazards.

29 CFR 1910.1200(f)(6): The employer used written materials, such as signs, placards, process sheets, or batch tickets in lieu of affixing labels to individual stationary process containers that failed to identify the container(s) to which they were applicable and did not convey the information required by 29 CFR 1910.1200(f)(5):    (a) Watson Mills and Design, LLC. at 1589 S. 120th St, Lafayette, CO: On and before April 5, 2017, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemicals contained within. Employees were potentially exposed to unlabeled and mislabeled hazardous chemicals including but not limited to acetone, SP Green Dye Concentrate, Chroma-Chemlamp Black and Presidio HS White CV Primer.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) Watson Mills and Design, LLC. at 1589 S. 120th St, Lafayette, CO: On and before April 5, 2017, the employer did not provide employees with effective information and training on hazardous chemicals in their work area.  Employees did not know the hazards of the chemicals in their work areas in that the employer did not provide an explanation on the work place labeling system. Employees were potentially exposed to unlabeled and mislabeled hazardous chemicals including but not limited to acetone, SP Green Dye Concentrate, Chroma-Chemlamp Black and Presidio HS White CV Primer.

29 CFR 1910.134(d)(1)(ii): The employer did not select and use a NIOSH-certified respirator in compliance with the conditions of its certification:  (a) Watson Mills and Design, LLC. at 1589 S. 120th St, Lafayette, CO: On and before April 5, 2017, the employer did not ensure that employees were provided NIOSH-certified filtering facepiece respirators. Employees conducting woodwork activities were provided The Safety Director 305040 Disposable Dust Masks  for voluntary use but the air purifying respirators were not NIOSH certified. This condition exposed employees to respiratory hazards.

29 CFR 1910.157(b)(1): A written emergency evacuation plan, meeting the requirements of 29 CFR 1910.38, was not developed and implemented  where the employer elected to evacuate employees upon sounding of the fire alarm:   (a) Watson Mills and Design, LLC. at 1589 S. 120th St, Lafayette, CO: On and before April 5, 2017, the employer elected to evacuate employees upon sounding of the fire alarm but did not develop procedures for evacuation in a written emergency action plan, meeting the requirements for 1910.38. This condition exposed employees to fire hazards.  Abatement Note: An emergency action plan must include at a minimum: procedures for reporting a fire or other emergency; procedures for emergency evacuation, including type of evacuation and exit route assignments; procedures to be followed by employees who remain to operate critical plant operations before they evacuate; procedures to account for all employees after evacuation; the name or job title of every employee who may be contacted by employees who need more information about the plan or an explanation of their duties under the plan.

29 CFR 1910.303(g)(1)(i): Wordspace was less than 3 feet in front of electrical equipment operating at 600 volts nominal, or less:  (a) Watson Mills and Design, LLC. at 1589 S. 120th St, Lafayette, CO: On and before April 5, 2017, the employer did not maintain a workspace of at least 3 feet in front of an electrical panel located on the western wall next to the CNC baghouse in the machine area in that the panel was blocked by a Rigid Blower Vac and wooden push broom. This condition exposed employees to electrical  hazards.   (b) Watson Mills and Design, LLC. at 1589 S. 120th St, Lafayette, CO: On and before April 11, 2017, the employer did not maintain a workspace of at least 3 feet in front of an electrical panel located on the eastern wall in the Finishing Area in that the panel was blocked by two wooden boxes containing wood and plastic materials and the base of a portable light. This condition exposed employees to electrical  hazards.
