On or about March 18, 2017 - at the above addressed jobsite - employees working in Unit 110 of a rehab building were exposed to falls, due a floor cover not being secured to prevent displacement, thereby exposing employees to fall hazards of approximately four (4) feet. b.) On or about April 13, 2017 - at the above addressed jobsite - employees working on the Third Floor near the trash chute were exposed to falls, due to covers not being secured to prevent displacement , thereby exposing employees to fall hazards of approximately ten (10) feet.
