RHKG Holdings, Inc. dba Maloney Plastics, 10940 Mercer Pike, Meadville, PA, Mold Maintenance area, on or about April 7th, 2017, and times prior thereto: Employees were exposed to electrical hazards in that, a flexible cord attached to the D-M-E brand controller was damaged and defected in that, portions of the outer jacket of the controller were damaged exposing the internal wiring of the controller.

29 CFR 1910.1200(h)(3)(iv): Employees had not been provided with Hazard Communication 2012 training including an explanation of the labels received on shipped containers and the workplace labeling system to be used by their employer; the safety data sheet, including the order of information, and how employees can obtain and use the appropriate hazard information: a.) RHKG Holdings, Inc. dba Maloney Plastics, 10940 Mercer Pike, Meadville, PA, on or about April 7th, 2017, and times prior thereto: Employees were working with and around hazardous substances including, but not limited to mold release, and mold saver. Some employees did not receive training on the new labeling elements and safety data sheet format noted in CFR 1910.1200 (j)(1) of the Hazard Communication 2012 Standard.
