29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights:   a)  Scaffold, East Exposure - On or about 01/18/2017, a stucco mechanic was exposed to a 50 foot fall from a scaffold platform which was not fully planked between the scaffold's vertical uprights.    No abatement certification or documentation required for this item.

29 CFR 1926.451(c)(1)(ii): Guys, ties, and/or braces were not installed  according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less thereafter for scaffolds greater than 3 feet (0.91m) wide:    a)  Scaffold, East Side of Building - On or about 01/18/2017, workers were exposed to a potential scaffold collapse from two fabricated frame scaffold towers which were 5 feet wide at their bases, 50 feet high and each secured with only a singe board located approximately at only 15 feet above each scaffold's base.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:   a)  Scaffold, East Side of Building - On or about 01/18/2017, workers were exposed to a 40 foot fall from the cross braces and frames of a fabricated frame scaffold while climbing up the scaffold components in order to access an elevated work platform.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
