OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an engulfment hazard while walking on an unstable walking surface on a wood chip pile:    (a)  Wood Chip Yard:  On or about April 8, 2017 and times prior, employees walked onto the wood chip storage pile near the reclaimer conveyor systems where the material could suddenly collapse from underneath them and pull them under the wood chip pile.    Among other methods, one feasible and acceptable abatement method to correct this hazard is to implement controls such as but not limited to work rules and warning signs in the hazardous areas.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to crushing injuries while riding on a dozer with no seat or seatbelt during training operations:    (a)  Wood Chip Yard:  On or about March 20, 2017 employees rode as passengers on the Allied Wagner CHD-24S Chip Dozer, equipped with only one seat.    Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure no employees other than the operator are permitted to ride on any mobile machine unless seating, seatbelts, and other protection equivalent to that provided for the operator is provided.  Also follow the Allied Wagner operations warning, Section 2.2 which states "The only person required on the machine is the operator.  Never allow anyone to ride on the machine or it attachments."

29 CFR 1910.29(b)(10): When a guardrail system was used at a hoist area, a removable guardrail section, consisting of a top rail and midrail, was not placed across the access opening when employees were not performing hoisting operations:    (a)  Lime Kiln Plant, Second Floor:  On or about April 19, 2017 a door to a hoist area was left open for ventilation and the wall opening only had a top rail for employee protection against an approximate fifteen foot fall hazard.

29 CFR 1910.23(b)(10): A ladder with structural or other defects was not immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with 29 CFR 1910.145 and removed from service until repaired in accordance with 1910.22(d), or replaced:    (a)  Number 2 Bark Belt, North Side:  On or about April 18, 2017 two Bauer 10-foot aluminum ladders used to gain access from the ground up to the first level had severely damaged side rails and were not taken out of service.    (b) Number One Bark Belt:  On or about April 18, 2017 a Bauer 10-foot aluminum ladder used to gain access from the ground up to the first level had a severely damaged support leg and was not taken out of service.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    (a)  Lime Kiln Plant, Ground Floor:  On or about April 19, 2017 an eyewash for emergency use was covered in a thick layer of lime from the process creating a corrosive eye hazard if used by employees.
