29 CFR 1904.32(a)(1): A review of the OSHA Log of Recordable Work-Related Injuries and Illnesses (OSHA Form 300 or equivalent) was not completed to ensure the Log was accurate and complete, and/or deficiencies were not identified and corrected at the end of each calendar year. (a) On or about January 29, 2018 the OSHA-300 log for calendar year 2017 was not reviewed correctly to ensure that the log was accurate prior to creating and certifying the 300A summary and posting it. The only entry on the 2017 300 log did not have the entry designated as an injury or illness in columns M1 through M6 on the log.
