29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  At 705 Lori Lane, Opelika, Alabama:  On or about April 11, 2017 and at times prior; the employer allowed his employees to use an extension ladder to gain access to the second floor and the rails of the ladder did not extend at least three feet above the landing area exposing his employees to a fall hazard of 10.1 feet to the concrete floor below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At 705 Lori Lane, Opelika, Alabama:  On or about April 11, 2017 and at times prior; the employer allowed his employees to walk the tops of framed walls without fall protection exposing his employees to a fall hazard of 19.5 feet to the ground below.  b)  At 705 Lori Lane, Opelika, Alabama:  On or about April 11, 2017 and at times prior; the employer allowed his employees to walk the tops of ceiling joists without fall protection exposing his employees to a fall hazard of 9 feet to the floor below.  THE GLZ  AND SON CONSTRUCTION, LLC  WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1215638 CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON APRIL 7, 2017, WITH RESPECT TO A WORKPLACE LOCATED AT 802 W RICHLAND RD., LOT 1, AUBURN,ALABAMA AND IN OSHA INSPECTION NUMBER 969130 CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON JULY 10, 2014, WITH RESPECT TO A WORKPLACE LOCATED AT LOT 15 & 16, 1673 EASTON CT., AUBURN ALABAMA.
