29 CFR 1910.178(a)(4): Modifications and additions which affected capacity and safe operation of powered industrial trucks were performed by the employer without the manufacturer's prior written approval:  DK6 Building, 1650 Macon Drive, DeKalb, IL.  On or about April 11, 2017, operators of Raymond standup-reach trucks equipped with 42" forks raised and lowered palletized loads with 72" fork extensions when placing loads and retrieving them from industrial storage racks.  The manufacturer provided the employer with written approval for the use of 72" fork extensions on Raymond standup/reach trucks equipped with 48" forks.  The employer did not obtain written approval from the vehicle manufacturer, to use 72" fork extensions on standup/reach trucks equipped with 42" forks.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
