29 CFR 1910.25(b)(3): The employer did not ensure that the stairs had uniform riser heights and tread depths between the landings: On or about April 11, 2017, the employer provided stairs to the second floor above Extruder 2 and the employees were exposed to fall hazards and the employer did not ensure the stairs had uniform riser heights: a) 7.25-inches between step 4 and step 5; b) 6.5-inches between step 5 to step 6; c) 9-inches between step 6 to step 7; and d) 7.25-inches between step 7 and step 8.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source: On or about April 6, 2017, employees were exposed to pinch-point hazards when unjamming the Unground System and a rod is used at the 2-inch cleanout opening to move plugged material at the rotary vein airlock and the employer did not ensure the electrical energy for the blower (480V) and airlock (480V) were isolated prior to unjamming the material ahead of the airlock.
