29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: Savanna Energy Rig #413 Drill Site: On or about April 6, 2017; the electrical circuit breaker box at the end of the crew trailer had two unprotected openings. This condition exposed the employees to potential electrical shock hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that the strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.: Savanna Energy Rig #413 Drill Site: On or about April 6, 2017; the strain relief to the AC unit was missing the nut exposing the wires to a metal edge. This condition exposed the employees to electrical shock hazards.
