29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:    a.   On or about April 11, 2017, on the 4:12 pitch roof on the right side of the house, two employees installing lathing were exposed to an 11 feet fall hazard from the eave in that they were not protected by a fall protection system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a.   On or about April 11, 2017, on the right side of the house, the roof access ladder did not extend 3 feet above the roof eave at the upper landing surface of the roof, exposing lathing installation employees to an 11 feet fall hazard.

On or about April 11, 2017, on the right side of the 4:12 pitch first-story roof, a folded step ladder was leaned against the second-story wall, and it was used to install lathing on the upper areas of that wall, exposing employees to a 15 feet fall hazard over the eave.  b.   On or about April 11, 2017, on the left side of the 4:12 pitch first story roof, a folded step ladder was leaned against the second-story wall, and it was used to install lathing on the upper areas of that wall, exposing employees to an 11 feet fall hazard over the eave.

On or about April 11, 2017, on the right rear of the 4:12 pitch first-story roof the feet of a step ladder were placed down the slope of the roof in an unstable manner exposing an employee on the step ladder to a 15 feet fall hazard.

On or about April 11, 2017, on the left side of the house, a 20-foot Werner aluminum extension ladder had a broken foot on the bottom of the right base section rail, and it had not been removed from service.
