29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  a) On or about June 12, 2018 the employer has failed to provide verification of abatement for below citation and penalty   29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) On or about September 6, 2017: Employees were installing wood framing elements on roof trusses and cutting truss ends on a residential structure and were exposed to a fall hazards of 12 to 18-feet and the fall protection training certification was not provided.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) On or about September 6, 2017: an employees was observed on the roof trusses at the back of the house cutting the ends of the trusses with a circular saw and was not wearing safety glasses, the employee was exposed to eye injuries.   b) On or about September 6, 2017: two employees were observed on the roof trusses at the peak of the house installing wood elements on trusses with pneumatic nail guns and were not wearing safety glasses, the employees were exposed to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about September 6, 2017: two employees were observed on the roof trusses at the back of the house cutting the ends of the trusses to install the fascia and were not protected by guardrail system, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure, the employees were exposed to a 12-feet fall hazard.     b) On or about September 6, 2017: two employees were observed on the roof trusses at the peak of the house installing wood elements on trusses and were not protected by guardrail system, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure, the employees were exposed to a 16-feet fall hazard.     Hernandez Drywall, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1229379, citation number 1, item number 1 and was affirmed as a final order on July 17, 2017, with respect to a workplace located at 128 Paradise Valley Drive Ponte Vedra, FL 32081.  Hernandez Drywall, LLC previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1224639, citation number 1, item number 1 and was affirmed as a final order on September 20, 2017, with respect to a workplace located at 88 Miranda Place Saint Johns, FL 32259.
