29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Sauder Woodworking Co. Archbold, Ohio; On or about April 6, 2017 the employer did not ensure that the ME7355 Black Brothers Roller Press was guarded to protect employees from in going nip point hazards. Employees were exposed to caught in injuries from an in going nip point created between a powered stainless roller and a powered rubber roller that coats light weight wooden panels with glue.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Sauder Woodworking Co. Archbold, Ohio; On or about April 6, 2017 employees were exposed to caught-in hazards relating to an ingoing nip point in that the ME7355 Black Brothers Roller Press was not locked out prior to cleaning glue coater rolls. The employer did not ensure that energy control application steps to control the machines electrical sources were implemented prior to servicing tasks such as but not limited cleaning, including: - Turn off or shut down the equipment. - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and de-energization of the machine or equipment had been accomplished.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) Sauder Woodworking Co. Archbold, Ohio; On or about April 6, 2017 the employer did not ensure that employees who service and maintain the ME7355 Black Brothers Roller Press were adequately trained in the skills required for the safe application, usage, and removal of energy controls. Employees that cleaned the Roller Press were not trained to control the hazardous energy sources of the machine during cleaning procedures.
