The employer did not provide a safe means of access and egress while unhooking a two leg sling with j hooks from a 5th wheel part while standing on top of the frame of a metal rack approximately 31 inches in height.

29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. The employer used a forklift beam lifting attachment secured to the tine of a Caterpillar sit-down forklift without the truck being marked to show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.

29 CFR 1910.184(e)(3)(i): In addition to the inspection required by paragraph (d) of this section, a thorough periodic inspection of alloy steel chain slings in use shall be made on a regular basis, to be determined on the basis of (A) frequency of sling use; (B) severity of service conditions; (C) nature of lifts being made; and (D) experience gained on the service life of slings used in similar circumstances. Such inspections shall in no event be at intervals greater than once every 12 months. A two leg alloy steel chain sling used in conjunction with a Caldwell Group forklift beam attachment on a forklift to lift parts into a metal rack was last inspected on November 15, 2015.
