29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:        a) On or about 04/13/17, on the left side (when looking at the front of) a house at 16 Sterling Place, Lancaster, NY:  Two employees were standing on the 14 inch wide aluminum "pic" of a ladder jack scaffold while installing the bottom layer of ice shielding on the upper roof. The employees were exposed to a fall of approx. 17 ft. to the ground. No fall protection system was utilized.         NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      a) On or about 04/13/17, on the upper roof on the left side (when looking at the front) of a house at 16 Sterling Place, Lancaster, NY:  Two employees engaged in roofing work on a steep roof were exposed to a fall of approx.  19 ft.  No fall protection system was utilized.  Pitch of that upper roof was approx.  10 in 12.      NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:      a) On or about 04/13/17, on the left side (when looking at the front of) a house at  16 Sterling Place, Lancaster, NY:  Employees used an extension ladder to access the  14 inch wide aluminum "pic" on the ladder jack scaffold.  The side rails of that ladder only extended approx.  24 inches above the "pic.        NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:      a) On or about 04/13/17, on the roof installation jobsite on a house at 16 Sterling Place, Lancaster, NY:  Employees engaged in construction work including roofing & clean-up were not trained to recognize potential hazards on this type of jobsite, including but not limited to falls from roofs, ladders, skylights,   scaffolds, electrical hazards, unguarded tools/machines, fire hazards, and the need for personal protective equipment to protect them from various hazards. They also were not trained in the applicable OSHA regulations to control or eliminate these hazards.      ABATEMENT CERTIFICATION REQUIRED
