Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) On or about 03/27/2025 at the site located at 5428 North Burdick Street, Fayetteville, New York: Employees were applying a new roof system on flat roof without the use of fall protection. Employees were exposed to falls of approximately 8 to 24 feet. Kris O'Connor DBA O'Connor Contracting was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1724943, citation number 1, item number 1 and was affirmed as a final order on 1/10/2025,with respect to a workplace located at 950 Broadway Street, Buffalo, New York.
