29 CFR 1910.165(e): Manually operated actuation devices for use in conjunction with employee alarms were not unobstructed, conspicuous and readily accessible. Gear issue room, on or about 12 April 2017 and times prior thereto; the manually operated fire pull alarm device located to the left of the doorway was obstructed. A hand truck and table was stored in from of the panel. This condition exposed employees to smoke and fume inhalation and fire hazards.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment. Building 2, on or about 12 April 2017 and times prior thereto; two instances were noted here electrical panels were obstructed: a. Gear issue room, a box was stored front of electrical panel labeled F, impeding access. b. Aerobic room, electrical panel labeled 8, had a rack of pads stored in front of the panel. This condition exposed employees to an electrical shock and electrical burn hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Building 2; on or about 12 April 2017 and times prior thereto; two instances were noted where electrical cords were damaged: a. Gear issue room, a National Brand, Electrical Inflator, Model 3190, Serial Number 12980005280 was damaged. The outer jacket protective insulation was separated from the plug on of the power cord. b. Cardio room, an Air King High Velocity floor fan, serial number unknown, was missing its ground pin. This condition exposed employees to electrical shock and electrical burn hazards.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit". Gear issue room, on or about 12 April 2017 and times prior thereto; exit signs were not posted at the doorways leading from the gear issue room to the reception/check-in area; and from the gear issue room to the break area/office areas. This condition exposed employees to delays in exiting the building, smoke inhalation, fire, and tripping hazards. The Coast Guard Station Kodiak MWR was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.37(b)(2), which was contained in OSHA inspection number 1003736, citation number 1, item number 1 and was affirmed as a final order on 08 December 2014, with respect to a workplace located at Coast Guard Station Kodiak MWR located at Kodiak, Alaska.
