29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: Battery Space: On or about July 11, 2019 and at times prior thereto, an employee with facial hair (beard) wore a respirator with a tight-fitting facepiece. This condition exposed the employee to inhalation hazards. General Industry standard applicable to shipyard employment.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting: Sheet Metal Shop: On or about July 11, 2019 and at times prior thereto, a four (4) inch rotating flange/shaft on the Bickford Chipmaster Radial Arm Drill was not guarded. This condition exposed employees to caught-in hazards. General Industry standard applicable to Shipyard employment.

29 CFR 1910.303(b)(6): Conductors or equipment were located in damp or wet locations; where exposed to gases, fumes, vapors, liquids, or other agents that had a deteriorating effect on the conductors or equipment; or where exposed to excessive temperatures: Blast Pot Door Area: On or about July 11, 2019 and at times prior thereto, the 208 volt welder connection box was exposed to conditions which limited its function. The corrosive environment prevented the connection box from closing completely. This condition exposed employees to contact with the 208 volt circuit.

29 CFR 1910.303(f)(1): Each disconnecting means required by Subpart S of Part 1910 for motors and appliances was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident: Sponge Blaster Room: On or about July 11, 2019 and at times prior thereto, the 480 volt disconnect was not legibly marked to indicate its intended purpose. This condition limited employee information in the event a facility component had to be deenergized. General Industry standard applicable to Shipyard employment.
