29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment.   Chemical room:  On or about 12 April 2017 and times prior thereto, the employer had not performed a hazard assessment on the handling of Accu-tab Chlorine tablets per the Safety Data Sheet.  Personnel are not required to don goggles and face shields while handing product.  This exposed employees to eye trauma.

29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:    Pool Area:  On or about 12 April 2017and times prior thereto, an emergency exit light were inoperative. This exposed employees to delays in existing the building causing smoke inhalation and fire hazards.      The Coast Guard was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(a)(4), which was contained in OSHA inspection number 1003757, citation number 1, item number 1 and was affirmed as a final order on 08 December 2014, with respect to a workplace located at USCG Base Kodiak.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  Recreation Center:  On or about 12 April 2017and times prior thereto, employees were not trained on the new Hazard Communications Standard (HCS 2012) provisions implemented on 1 December 2013. This training includes revised labeling elements and the new format for Safety Data Sheets. This condition exposed employees to inadequate training and chemical hazards.  The Coast  Guard was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(h)(3)(iv), which was contained in OSHA inspection number 1003708, citation number 2, item number 1 and was affirmed as a final order on 08 December 2014, with respect to a workplace located at USCG Base Kodiak.
