Pizza Parlor, Hallway to back room: On or before 12 April 2017 and times prior thereto, the Beacon/Morris mounted did not have a fan guard installed. Fan measured approximately 80 inches off of the floor. This condition exposed employees to a caught in hazard)

29 CFR 1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater Pizza Parlor, Store room: On or about 12 April 2017 and times prior thereto, electrical panel C was blocked. This condition exposed employees to an electrical shock and burn hazard.
