29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch.  Welding room; on or about 13 April 2017 and times prior thereto; a Dewalt Model DW758 Bench Grinder; Serial Number 201314-YL87032, tool rest was not properly adjusted to a maximum of one-eighth of an inch from the abrasive wheel. The gap between the abrasive wheel and work rest measured approximately one-quarter of an inch. This condition exposed employees to a caught in hazard.
