29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch. Welding room; on or about 13 April 2017 and times prior thereto; a Dewalt Model DW758 Bench Grinder; Serial Number 201314-YL87032, tool rest was not properly adjusted to a maximum of one-eighth of an inch from the abrasive wheel. The gap between the abrasive wheel and work rest measured approximately one-quarter of an inch. This condition exposed employees to a caught in hazard.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment. Bay 7, on or about 13 April 2017 and times prior thereto; an electrical panel labeled D, had a Snap model floor creeper stored in front of the panel impeding immediate access. This condition exposed employees to an electrical shock and electrical burn hazard.
