29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: (a) On 4/17/2017, in the paint preparation area were employees are required to wear full face tight fitting respirators with combination cartridge, a respirator was hanging on a hook uncovered, when employee was asked how long it had been their the employee stated it had been there for two weeks. The employer failed to ensure that respirators were stored in a manner to protect them from damage.

29 CFR 1910.134(g)(1)(i): 29 CFR 1910.134(g)(1)(i): The employer permitted respirators with tight-fitting facepieces to be worn by employees who have facial hair that came between the sealing surface of the facepiece and the face or interfered with valve function: (a) On 6/9/2017, in the paint shop and vacuum pressure impregnation area were employees are required to wear full face tight fitting respirators with combination cartridges to apply spray coatings and degreasers containing solvents such as but not limited to Wash Primer Catalyst RK7K44, Xylene, Acetone, Vinyl Toluene and Safety Kleen Heavy Duty Lacquer Thinner, employees had facial hair that came between the sealing surface of the facepiece.
