29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times.  Building 26, warehouse, walk-in freezer; on or about 13 April 2017 and times prior thereto; the emergency escape plunger on the interior side of the walk-in freezer door was inoperative. Emergency escape plunger will not cycle preventing egress. This condition exposed employees to an entrapment hazard.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  Building 26, Sporting goods area; on or about 13 April 2017 and times prior thereto; two energized power taps were daisy chained together. A 3-foot, Power Source Model PS600 power tap was plugged into a 6-foot, Tripp Lite 6-place, Model E128811 power tap. This condition exposed employees to fire hazard.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment.  Building 26, on or about 13 April 2017 and times prior thereto; four instances were noted where electrical panels were obstructed:       a. Receiving area, a cart with miscellaneous items and boxes were stored front of electrical panel labeled 2LB, impeding access.       b. Receiving area, a small refrigerator, file cabinet, microwave, and miscellaneous items were stored in front of electrical panel labeled TT21.       c. Sporting good sales area, two electrical panels, labeled 2LA and 2HA had items stored in front of them impeding access. A portable stand with electronic equipment was stored in front of panel labeled 2HA. A stool was stored in front of panel labeled 2LA.  This condition exposed employees to an electrical shock and electrical burn hazard.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition.  Building 26, warehouse area; on or about 13 April 2017 and times prior, the employer did not ensure that the work areas were clean and orderly. Warehouse area had numerous items loosely scattered on the floor impeding the walking and working surface. This condition exposed employees to a tripping hazard.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6.  Building 26, adjacent to registrar counter, on or about 13 April 2017 and times prior thereto; a compressed gas cylinder containing helium was not secured. This condition exposed employees to a struck by hazard.
