29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: a) On or about April 12, 2017: on a 4:12 pitch roof employees were observed cleaning the roof to install the underlayment membrane and were using a safety monitor as fall protection. The safety monitor was performing other tasks such as sweeping the roof and removing nails, the employees were exposed to an 8-foot fall hazard.

The safety monitor was performing other tasks besides been the safety monitor and was not able to explain the criteria when a safety monitor is needed or its responsibilities.
