29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: a. On or about June 27, 2017, roof area, three employees were observed moving shingles and were using a safety monitor as fall protection. The safety monitor was performing other tasks such as moving shingles. The employees were exposed to a 10 feet fall hazard.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a. On or about June 27, 2017, employees operating nail guns were not wearing safety glasses and were being exposed to flying debris and nails. Jim Taylor Roofing, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1133325, citation number 01, item number 001 and was affirmed as a final order on May 7, 2016, with respect to a workplace located at 1511 Derbyshire Road, Daytona Beach, FL 32117.
