29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    The employer did not develop and implement an energy control program consisting of energy control procedures, employee training, and periodic inspections for employees engaging in cleaning and sanitation activities on equipment located at 300 Moore Rd. Plover, WI 54467.

29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  The energy control procedure outlined in page nine of the employee manual did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance when employees were performing cleaning and sanitation activities on equipment located at 300 Moore Rd. 29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    On or about April 10, 2017, an employee was exposed to the ingoing nip point between the rotating drum and the doctor blade while cleaning an Idaho Steel Nex-Gem potato extruder located at 300 Moore Rd. Plover, WI 54467.  The employer did not ensure employees implemented energy control application steps prior to them engaging in cleaning and sanitation activities of the extruder.  The machine was not deenergized prior to cleaning and sanitizing of the drum area of the extruder.      As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing and maintenance hazards.
