29 CFR 1910.30(a)(3)(iv): The employer did not train each employee in the correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of this section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer: (a) Warehouse area; On or about December 19, 2023, and at dates prior, the employer did not adequately train employees in the correct use and limitations of personal fall protection systems. The methods used to anchor personal fall arrest systems during elevated work in pallet racking systems bypassed factory deceleration devices and subjected the retractable web lanyards to additional damage/wear from the snaphook being attached directly to the webbing.

29 CFR 1910.140(c)(10)(i): The employer did not ensure that fall protection system snaphooks and carabiners are not connected directly to webbing, rope, or wire rope unless they are designed for such connections: (a) Warehouse area; On or about December 19, 2023, and at dates prior, snaphooks of retractable lanyards were connected directly to the lanyard webbing to make personal fall arrest system anchorage points around overhead members of a pallet racking system during elevated cleaning operations in the racking systems.

29 CFR 1910.140(d)(1)(i): The employer did not ensure that personal fall arrest systems limit the maximum arresting force on the employee to 1,800 pounds (8 kN): (a) Warehouse area; On or about December 19, 2023, and at dates prior, arresting forces on personal fall arrest systems were not limited because the factory installed deceleration devices installed directly between the snaphook and retractable web portion of the lanyards was bypassed when the lanyards were wrapped around overhead members of a pallet racking system and clipped back onto the lanyard to make anchorage points.
