29 CFR 1910.137(c)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests. Test voltages and/or the maximum intervals between tests were not conducted in accordance with Table I-4 and Table I-5.    On or about April 17, 2017, employee(s) performing work at various field locations were required to use electrical protective equipment such as rubber insulating gloves during their assigned electrical work. The employees were required to work on or near voltages between 110 VAC - 480 VAC. The rubber insulating gloves were not subjected to periodic electrical tests. Employees were thereby exposed to the hazards associated with live electrical parts.

Western Springs, IL. And other field repair locations - On or about April 17, 2017, a motor technician  was required to troubleshoot the circuit elements and electrical parts of a well pump located at 740 Hillgrove Ave. Western Springs, IL., Pump House, Well No. 3. The employer's procedures did not include step corresponding to the requirements of paragraph 29 CFR 1910.333(b)(2)(iv).

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:    Western Springs, Illinois - On or about April 17, 2017, a motor service technician was required to troubleshoot the circuit elements and electrical parts of the 2,300 V AC well pump located at 740 Hillgrove Avenue, Western Springs, IL., Pump House, Well No. 3. The technician worked in an area where there were electrical hazards (2,400 V AC) and was not provided with electrical protective equipment including voltage-rated gloves and flame resistant clothing. The employee was thereby exposed to the hazards associated with live electrical parts.
