29 CFR 1910.304(g)(3)(i): The employer did not ensure the frame of a portable generator was grounded and served as the grounding electrode for a system supplied by the generator that was not mounted on the generator or cord- and plug-connected equipment through receptacles mounted on the generator, or both; and the noncurrent-carrying metal parts of the equipment and the equipment conductor terminals of the receptacle was bonded to the generator frame: a) Colorado Springs drilling site: On or about May 17, 2022, and at times prior, workers were exposed to electrical shock hazards while the trailer-mounted Taylor TD350 generator supplied power to a physically distant shale shaker table that was not electrically grounded and bonded to the generator. b) Colorado Springs drilling site: On or about May 17, 2022, and at times prior, workers were exposed to electrical shock hazards while the shale shaker table conductor was not connected directly to the Appleton 480v cannon plug receptacle mounted on the exterior of the Taylor TD350 generator.

29 CFR 1910.305(d)(2): Panelboard enclosures. Panelboards were not dead front: a) TD350 generator at drilling site: On or about May 17, 2022, and at times prior, workers were exposed to electrical shock through direct contact with energized 480 volt conductor components in a TD350 electrical panel that was not protected through the installation of a dead front panel cover while to TD350 electrical panel access door remained open.
