29 CFR 1910.95(g)(8)(ii)(B): Unless a physician determines that the standard threshold shift is not work-related or aggravated by occupational noise exposure, the employer did not ensure, when a standard threshold shift occurs, that for employees already using hearing protectors, employees be refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary: Production areas; On or about 5/3/17, employees who had standard threshold shifts were not refitted or retrained in the use of hearing protection and not provided additional hearing protection where necessary. The employer had four standard threshold shifts recorded on their 2016 log and additional shifts in previous years.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work shift to employees when they were in their work area(s) Production Area; On or about 4/24/17, the employer did not have GHS compliant safety data sheets for all the chemicals used at the facility. Safety data sheets that have not been updated include, but are not limited to the sds for Rust Preventative 6000 was last updated in 1993.
