29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a) At the Glue Machine (Production Area):  A portable Honeywell eye wash station was not maintained in a working condition (covered with dirt and other debris and did not provide for continuous 15 minutes of drenching of the eye) where employees were using corrosive chemicals such as Aquence Catalyst 7100 as observed on or about April 24, 2017.                                             ABATEMENT CERTIFICATION IS REQUIRED FOR THIS VIOLATION.

29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:    a) Production Area:  The employer did not provide the educational program to familiarize employees with the general principles of fire extinguisher use at least annually, as observed on or about April 24, 2017.                                            ABATEMENT CERTIFICATION REQUIRED FOR THIS VIOLATION.    Rad Manufacturing, LLC., dba Rad Engineered Wood Products was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(g)(2), which was contained in OSHA inspection number 1107729, citation number 2, item number 1 and was affirmed as a final order on March 1, 2016, with respect to a workplace located at 531 Maple St., Nescopeck, PA 18635.

29 CFR 1910.1200(h)(3)(iv): Employee training did not include the details of the hazard communication program developed by the employer, including an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employees can obtain and use the appropriate hazard information:      a) At the Facilty: Employees worked with hazardous chemicals, including, but not limited to, chemicals such as Aquence Catalyst 7100 and Aquence Adhesive 2163, and the employer did not provide these employees with the required training on the new label elements and safety data sheet format as required by the revised OSHA Hazard Communication Standard incorporating the Globally Harmonized System of Classification and Labeling of Chemicals prior to the effective date of December 1, 2013, on or about April 13, 2015.                                                                                                        ABATEMENT CERTIFICATION IS REQUIRED FOR THIS VIOLATION.
