29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. A) Exterior on Scaffold / Front of the house - An employee was exposed to fall hazards of approximately 21 feet while working on a ladder jack scaffold that was not provided with safe access/egress on to and off of it, on or about March 20th, 2017.

A) Exterior on Scaffold / Left side of house - Employees were exposed to fall hazards of approximately 21 and 25 feet while working on ladder jack scaffolds that were not protected by a complete guardrail system or any other means of fall protection in place, on or about March 20th, 2017. The Finest Construction, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 732941, citation number 1, item number 1 and was affirmed as a final order on December 12, 2012, with respect to a workplace located at 1538 River Road in Fort Lee, NJ.
