29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: (a): Exterior Roof - Personal fall arrest systems were not inspected prior to use and defective components were not removed from service. An employee fell approximately 30 feet while using defective fall arrest components. A damaged vertical lifeline and rope grab failed to properly arrest his fall, on or about July 9, 2019. There is no abatement certification or documentation required for this item.
