29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     a.)    On or about April 26, 2017, employees were exposed to struck by hazards when performing cleaning activities inside the Fanuc robot (Serial #F-95689) cell on the small bagging line. The employer did not ensure energy control procedures were documented and utilized to properly control the electrical energy before entering the cell.       b.)    On or about February 26, 2017, employees were exposed to struck by hazards when removing and replacing punctured animal feed bags inside the Fanuc robot (Serial #F-95689) cell on the small bagging line. The employer did not ensure energy control procedures were documented and utilized to properly control the electrical energy before entering the cell, in that the robot started up and completed a cycle while employees were inside the cell.           c.)    On or about April 26, 2017, employees were exposed to pinch points while performing cleaning activities in the bagging area (Accelpak AP2600) of the small bagging line. The employer did not ensure energy control procedures were documented and utilized to properly control the electrical energy before cleaning the area.           d.) On or about April 26, 2017, employees were exposed to caught by and crushing hazards while performing cleaning activities on the Sealer (Fischbein Bag Closing PBC-6000) on the small bagging line. The employer did not ensure energy control procedures were documented and utilized to properly control the electrical and pneumatic energy before cleaning the sealer.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:     a. On or about April 26, 2017, the employer did not ensure employees were adequately trained in the purpose and function of the energy control program. Employees were required to perform activities such as cleaning and clearing damaged feed bags inside the Fanuc Robot Cell on the small bagging line, and are exposed to struck by and caught in hazards from moving parts in that the equipment was not properly deenergized.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     a.)    On or about April 26, 2017, the employer did not ensure employees were protected while working around the Fanuc Robot (Serial #F-95689) on the small bagging line in the Warehouse. Employees were required to work around the robot on the bagging line, and are exposed to struck by hazards from the moving robot in that it is not adequately guarded.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:     a.)    On or about April 26, 2017, the employer did not ensure employees were protected while using compressed air for cleaning activities. Air nozzles used for cleaning purposes were not reduced to less than 30 psi for cleaning purposes to prevent a struck by accident and to prevent an air embolism.
