29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energization, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      Entire Facility.   On or about April 26, 2017 through May 12, 2017, employees performed servicing and maintenance when cleaning machines and equipment such as but not limited to vertical mixers, ovens and proofers.  The employer did not implement and maintain a lockout/tagout program consisting of specific energy control procedures, employee training and periodic inspections as follows:       a) Procedures for the control of hazardous energy were not developed, documented and utilized by the employer.       b) Training of "Authorized", "Affected" and "Other" employees was not conducted and certified by the employer.   Employees authorized to perform lockout/tagout were not informed of and trained to control hazardous energy sources according to specific procedures.      c) Periodic inspections of the energy control procedures were not conducted and certified by the employer at least annually to ensure that the procedures and requirements of the standard were being followed.      Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     Mixing Room.  Employees were exposed to the rotating shafts/mixing blades on the following mixers:     a)    The Hobart 80 quart vertical mixer (MN L800, SN1242374) did not have a guard.     b)    The Hobart 140 quart vertical mixer (MN HL190, SN80-03049), had an opening in the guard near the front.     c)    The Hobart 20 quart vertical mixer (MN D300, SN1361559) did not have a guard.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.263(g)(2): Handfed moulder(s) were not provided with either a belt-device or a hopper that extended high enough so that the hands of the operator could not get into the feed rolls:    Production Room, LVO Sheeter Moulder No. 528 (MN SM 24, SN SM24-0403-1040).  On or about May 12, 2017, employees operated a handfed moulder.  The guards on top of the moulder machine at the opening to the feed rollers were not adequate to prevent entry of the operators' hands/fingers into the feed rolls.  Employees were exposed to degloving/crushing injuries.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.263(j)(1)(iii): Cover(s) over the knife head of reciprocating blade slicer(s) were not provided with an interlocking arrangement so that the machine could not operate unless the cover was in place:    Packaging Area, Globe 3/4" reciprocating blade slicer (Berkel MN MB7-16, SN 9213-3026-04816).  On or about May 12, 2017, employees could not operate the slicer without placing their hands near the reciprocating blades in order to hold the bread in place against the blades at the beginning of the cut.  The operating lever on the side of the machine was broken and not functioning.  Employees were exposed to amputation and laceration hazards.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:       Packaging Area, Near Bread Slicers, on or about May 12, 2017. Two relocatable power taps (temporary power strips), which were not designed for use in an industrial environment, were used to supply power to several bread slicing machines and stand fans.  Employees were exposed to overloading and overheating of electrical components that could cause a fire.       Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.305(g)(1)(iv)(D): Flexible cords and cables were attached to building surfaces.    Dock Area.  On or about April 26, 2017, flexible cords that were used for charging powered pallet jacks and powering a pest control light were run across building walls and attached to conduit with zip ties, in order to be connected to an outlet box.    There is no abatement documentation or certification required for this item.
