29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):   On or about April 26, 2017, employees at Red Hen Bread, LLC., were required to mix a solution of sodium hydroxide from solid sodium hydroxide.  The employer did not assess the workplace to determine the hazards associated with the handling of sodium hydroxide.  Employees were not provided with the appropriate personal protective equipment and were exposed to chemical burns from the sodium hydroxide.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.138(b): The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified:  On or about April 26, 2017, employees at Red Hen Bread, LLC., were required to mix a solution of sodium hydroxide from solid sodium hydroxide.  The employer did not assess the workplace to determine the hazards associated with the handling of sodium hydroxide.  Employees were not provided with the appropriate personal protective equipment and were exposed to chemical burns from the sodium hydroxide. 29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  On or about April 26, 2017, employees at Red Hen Bread, LLC., were required to perform maintenance on a forklift battery by adding water to it.  Employees were exposed to the corrosive battery acid and were not provided with suitable facilities, in the immediate work area, for quick drenching or flushing of the eyes and body.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
