29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  On June 15, 2017, Ed Herschberger, dba Herschberger Builders was issued citations and the employer did not verify, through written certification that each citation was abated.  Abatement certification was not received for the following violations:          a)  Citation 1 Item 1, and     b)  Citation 1 Item 2, and     c)  Citation 1 Item 3, and     d)  Citation 1 Item 4.  Ed Herschberger, dba Herschberger Builders,  was previously cited for a violation of this occupational safety and health standard or its equivalent standard , 29 CFR 1903.19(c)(1) which was contained in OSHA inspection number 1114209, citation number 2, item number 1 and was affirmed as a final order on 05/12/2017, with respect to a workplace located at 1209 Morningside Lane, Mahomet, IL.

29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.   On  February 5 2016, Elmer Miller dba Miller Building Systems, LLC, was issued citations and the employer did not verify, through documents that each citation was abated.  Abatement documentation was not received for the following violations:    On June 15, 2017, Ed Herschberger, dba Herschberger Builders was issued citations and the employer did not verify, through documents that each citation was abated.  Abatement certification was not received for the following violations:            Citation 2 Item 1        Ed Herschberger, dba Herschberger Builders,  was previously cited for a violation of this occupational safety and health standard or its equivalent standard , 29 CFR 1903.19(d)(1) which was contained in OSHA inspection number 1114209, citation number 2, item number 1 and was affirmed as a final order on 05/12/2017, with respect to a workplace located at 1209 Morningside Lane, Mahomet, IL.

29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:      On or about April 25, 2017, Ed Herschberger dba Herschberger Builders had two employees exposed to fall hazards in excess of 10 ft., while working from a supported scaffold which was lifted by a rough terrain forklift, on a residential construction site in Champaign, IL.  The supported scaffold was not secured to the forklift mast by a chain or some other secure method.
