29 CFR 1926.100(a):  Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:       On or about April 25, 2017, at the trenching site located at 1728 E. 14th Place in Tulsa OK, an employee did not wear protective head gear while working in a trench more than 8 feet deep, exposing the employee to being struck by falling/collapsing soil.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:      On or about April 25, 2017, at the trenching site located at 1728 E. 14th Place in Tulsa OK, employees were allowed to work in a trench more than 4 feet deep that had no ladder or other safe means access/egress.   Employees were exposed to struck-by and crushed-by hazards.
