29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  On or about April 25, 2017, employees were not trained to avoid unsafe situations. An employee was struck by a piece of plywood that fell from the roof.  A crew was installing plywood on the flat roof on the second level at the leading edge. Another crew decided to start work below them. Subsequently a piece of plywood fell down and struck the employee on the hard hat.  Employees were exposed to a struck-by hazard. The plywood was measured at approximately about 48 inches by 20 inches and fell approximately 15 feet.
