San Antonio, TX.: On or about August 24, 2021, and at times prior thereto, workers were exposed to fall hazards while using the top rails of an extended scissor lift as a platform to perform framing installation.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: 14250 Potranco Rd. San Antonio, TX.: On or about August 24, 2021, and at times prior thereto, workers were exposed to fall hazards while standing on the top rail of an extended scissor lift while performing framing installation to a commercial building, without recognizing the hazards of falls.
